Aims
The main aim of Diana Princess of Wales Care at Home Trust is to provide high quality nursing care and support the individual patient, the family, friends and carers, when the patient has chosen to remain at home. In the case of a child, when additional care is required or the family need a break.
Who can benefit from the service?
Patients who have a life limiting illness, for example, cancer, other progressive diseases and where curative treatment is no longer possible or appropriate.
How much will it cost?
Care is provided free of charge to patients resident in the Isle of Man.
Who provides the care?
Care is provided in the patient's home by Registered Nurses including Registered Sick Children's Nurses and experienced Health Care Assistants and Nursery Nurses, trained in palliative care, working in partnership with other professionals.
How can I or my family receive this help?
Your GP, Consultant, District Nurse, Health Visitor or Social Worker may refer you to the Palliative Care Specialist Team based in Hospice Isle of Man. One of the Clinical Nurse Specialists will make an assessment and organise your care.
Does the service include children?
Yes, the service supports children with life limiting conditions and their families, both emotionally and practically, by providing respite care in their own homes.
How is the service funded?
The service is supported by the Diana Princess of Wales Care at Home Trust and the service is provided by Hospice Isle of Man.
